The GMM Trust will limit sharing of the information of the patient as needed. Every patient’s medical records like blood work and prescriptions, contact details, addresses, family details, emails, and phone numbers are protected and not disclosed to the public.

By proceeding with the transaction Donor understands that the transaction is final and once the payment is made, then the donor is not eligible for any claims. Further, the donor confirms that all personal details provided in this form are true and correct. Donor further agrees to compensate GMM Trust against any and all claims, losses, and damages that may arise out of false or incorrect information submitted by the donor.
